Summary:
The Nurse Case Manager will provide care management for patients within the senior care program. The Nurse will provide quality patient care dependent upon each individuals specific needs. Hours are 8a-4p, Monday through Friday.

Responsibilities:

• Develop care plans based on each individuals needs and the physician’s care goals.
• Conduct visits to all patients to reassess frequency and services needed.
• Record and maintain accurate documentation.
• Oversee implementation of patient care plans.
• Frequent local travel

Requirements:

• Current RN license in Massachusetts, or ability to obtain before start date
• Current CPR Certification
• Covid and Flu Vaccinations
• 2 years of Home Care experience preferred
