Summary:

The Social Worker will plan, implement and evaluate care plans to ensure proper care within the PACE program. Hours are Monday-Friday, 8a-4p.

Responsibilities:

• Collaborate with the interdisciplinary team to evaluate a patients needs, form a care plan and reevaluate when necessary.
• Facilitate hospital, rehab and nursing home admissions and discharges as needed.
• Arrange and facilitate family meetings when necessary.
• Assist in transitioning patients from home living to long term care when needed.
• Assess psychological needs of patients with the help of behavioral health providers.

Requirements:

• Current license in Massachusetts as a Social Worker
• Current L.C.S.W or L.I.C.S.W
• Elder experience preferred
• Current CPR Certification or ability to obtain
• Frequent local travel with access to personal vehicle
• Covid and Flu Vaccinations required
• Fit Test Required
