Family Nurse Practitioner
Salary: $100,000-$110,000/yr based on experience
Summary:
Seeking a Family Nurse Practitioner (FNP) to join a non-profit health center which has six locations in Northern New Hampshire. They provide affordable, high-quality healthcare to nearly 10,000 patients from over 26 towns, offering medical, dental, and behavioral services for families. Everyone is welcome, and no one is turned away due to inability to pay. The center receives federal funding and has special protections for health-related claims, including medical malpractice.
Responsibilities:
- Perform thorough patient assessments and physical exams.
- Diagnose and treat both acute and chronic health issues.
- Create and execute personalized care plans for patients.
- Prescribe medications and other necessary treatments.
- Offer preventive services, such as immunizations and health screenings.
- Educate patients and families on maintaining health and preventing disease.
- Work with physicians and other healthcare providers to ensure coordinated care.
- Keep accurate and updated patient records.
- Engage in community outreach and health promotion initiatives.
- Stay informed about medical advancements and best practices.
Requirements:
- Master’s or Doctoral degree in Nursing from an accredited institution.
- Certification as a Family Nurse Practitioner (FNP) or equivalent.
- Current APRN license to practice in New Hampshire.
- Strong clinical skills and understanding of primary care.
- Excellent communication and interpersonal abilities.
- Dedication to delivering high-quality, patient-centered care.
- Ability to work effectively in a team-oriented setting.
- Experience in community health or with underserved populations is a plus.
