Summary:
As a member of the PACE Interdisciplinary Team in Adult Day Health Center setting, the Registered Nurse participates in the planning, implementation, and evaluation of care plans that meet the objectives, standards, and policies of the PACE model of care. New grads and experienced nurses encouraged to apply! Hours are 8a-4p, Monday through Friday.

Responsibilities:

• Provide direct care to participants
• Complete initial and periodic assessments; develop and revise care plans based on emergent and pre-existing needs
• Collect medical and social information to ensure comprehensive decisions for participant safety and well-being
• Identify and prioritize participants problems/needs; handle immediate referrals and follow-ups; interpret and record diagnostic results
• Coordinate with the interdisciplinary team to develop comprehensive care plans
• Ensure all tests, lab work, and diagnostic studies are completed
• Coordinate medication management
• Arrange delivery of medications and supplies to participants’ homes
• Provide education and consultation to participants and caregivers on care plans, treatments, and medications
• Communicate participants’ conditions to primary caregivers, physicians, and the multidisciplinary team
• Participate in after-hour visits as needed

Requirements:

• Current RN license in Massachusetts, or ability to obtain before start date
• Current CPR Certification
• Local travel with access to personal vehicle when needed
